6/19/2023 EnchantaVaughn Approved Behavior: Once the session began, Ashiyah was attentive and focused and displayed a high level of confidence when reflecting on her feelings. QMHP-Cengaged her in a discssion about her behaviors since the last session. She discussed with QMHP-C on how she lashed out on her grandmother the other day because she was coming out “slick” out the mouth. Behavior: During second portion of the session, Ashiyah was in a calm mood, as evidenced by her relaxed facial expressions and demeanor. Ashiyah was seen holding a conversation with her family and displaying appropriate manners. QMHP-C facilitated a family discussion to talk about common mistakes children with ADHD make and how she can make better choices. QMHP-C reviewed home expectations and how Ashiyah should conduct herself around peers and authority. QMHP-C encouraged Ashiyah to continue displaying age appropriate behavior, and the benefits of this will be reviewed with parent following each session. QMHP-C conducted a review of the previous week and asked Ashiyah what she learned from it. QMHP-C assisted Ashiyah with problem-solving skills. QMHP-C provided Ashiyah with positive feedback due to her responses. Ashiyah engaged in self-reflection and reflected on her day, the good and the bad in front of mom and seemed happier and more open minded with the interventions that were displayed. Ashiyah says please, thank you, and excuse me appropriately during the session. Ashiyah shows empathy towards mom, as evidenced by her hugging, playing, smiling, and complimenting her. Ashiyah avoids negative language (i.e. profanity words). Ashiyah recognized her triggers and was able to identify common interests and similarities she and other ADHD recipients shared. Ashiyah understand that she is expected to act civilized and use her inside voice while inside and transition safely to session and to places in the community that are safe and therapeutic. Ashiyah is observed making better choices and doing the right thing at home. Ashiyah has improved with staying seated without fidgeting behavior and utilized her breaks appropriately without displaying attention-seeking behaviors to get her way. Ashiyah responds well to praise and support. Ashiyah is acceptive of redirection. Ashiyah understands that her actions have consequences. Ashiyah is striving for a good report from mom and family every day.
